What they needed
Telemedicine sits at the intersection of strict regulation and real-time performance — there's little room for error in either direction. The platform needed live video consultations that stayed reliable under real consultation conditions, since a dropped or laggy call mid-appointment is a clinical problem, not just an inconvenience. Every layer also had to satisfy HIPAA: encrypted data in transit and at rest and secure handling of e-prescriptions and health records — obligations most consumer web apps never have to design around. On top of that came scale: the system had to support 100K+ active patients and their providers, with video, scheduling, and EHR data all moving through the same platform without one component's load degrading another.
What we built
We built the platform on a Django and PostgreSQL backend, which gave us the relational data models and access-control granularity HIPAA compliance requires, with React powering the patient- and provider-facing interfaces where clarity and responsiveness matter most during a live consultation. Video calls run end-to-end encrypted, so sessions stay private between patient and provider by design. Appointment scheduling connects directly to the EHR integration layer, keeping visit history, records, and e-prescriptions in sync. Docker containerized each core service — video, scheduling, records — so load in one area couldn't cascade into another. AWS provided the underlying infrastructure, with encryption enforced at rest and in transit throughout. The platform passed all compliance audits on first review.
How we approached it
Chose Django and PostgreSQL for the core backend to get relational integrity and fine-grained access control across patient records, prescriptions, and provider permissions in one auditable system.
Split video, scheduling, and records into separate Dockerized services, so a spike in video traffic during peak consultation hours couldn't degrade appointment booking or records access.
Enforced encryption in transit and at rest as a default across the AWS infrastructure from the start, rather than layering it on after launch, to support HIPAA compliance from day one.
Used React for the patient and provider interfaces to keep scheduling and in-call screens fast and responsive, since lag during a live video consultation is a clinical concern, not just a UX one.
